What To Bring
On this first visit you must bring:
- Cash, checkbook, or major credit card (Visa, MasterCard, Discover, American Express)
- Current dental insurance card or Medicaid card.
- Information for billing – Social Security number, date of birth, employer, group number.
- Photo I.D.
- The parking “spitter” ticket from the patient parking area of the structure for validation.
Patients under the age of 18 years must be accompanied by a parent or legal guardian.
Visitors and children are not allowed in the treatment area and must be supervised in the waiting areas of the School.
